The Daily Mail in London reports that the NHS has finally given the green light that botox can be used to treat migraines.  These botox injections will be offered on the NHS.  It says in the article: ‘the effectiveness of Botox (for migraines) was only discovered in the early 1990s when women in the US having jabs to smooth out wrinkles noticed their headaches and migraines ceased.’  Some US doctors performed botox trials that showed a 92% success rate.  In London, researchers have also performed botox trials, and found that ’70% of patients suffered 50% fewer migraines.’

Q. I’ve got some some lines around my mouth which make me look older than my age, what can I do to make them less visible? Would botox help?

A: These lines are usually exacerbated from smoking so if you are a smoker, stop immediately.  The easier treatment option is inject a temporary filler which will last 6 months to 1 year, and botox will not work for these lines unlike other lines on the face.  The other option is more invasive treatment to wipe out the lines via a laser treatment or chemical peel, however, there is a downtime of 2 weeks with possible side-effects.  After the treatment, make sure the skin is moisturized and a total sunblock should be added to the daily face routine.

Q:  I am desperate to loose some weight especially around the stomach and thigh areas, I am 5ft 5 inches and weigh 60 kg.  Is liposuction a good way to tackle this? 

A: VASER Liposuction is not a solution to weight loss.  It is actually to shape and contour the body.  First, it is important to get into a good healthy lifestyle in terms of diet and exercise.  Once you reached a stable weight, you can use VASER liposuction to remove stubborn areas such as the abdominal and thigh areas.  Alternatively, others are motivated by the liposuction; they see it as a kickstart and become motivated to keep their new shape.  The VASER liposuction technique is a good method where the patients recover quickly after a week, have relatively less bruising and pain compared with more traditional methods.

Q: I have ears that stick out. I moved to Dubai and the hot weather is forcing me tie my hair back more often, and I hate seeing my ears.  It is the first time I have considered something about them.  Please can you advise that plastic surgery involves?

A:  The surgery is called otoplasty.  It involves an incision of 2 cm at the back of the ears, and through the incision we are going to reshape the cartilage of the ear to soften it so the ear can be bent backwards, and fix the ear with some permanent sutures.  It is important for the first week to wear an ear band, day and night, and the following two weeks, the band just needs to be worn at night time.  The procedure is relatively painless but the ear band must be worn as instructed to prevent relapses, and close physical contact must be avoided for first couple of weeks.  This can be carried out under a mild sedation.

The disadvantage of the eye bag surgery

It is important to choose a good plastic surgeon for this procedure because the right amount of fat has to be removed, and not too much should be taken out otherwise the patient can get a hollow look under the eyes.

This can include using

Dermal fillers – to blend in the puffiness of the eye bag

Eye Creams – Very few of these creams work in extreme cases but it is investing in one to prevent the onset of eye bags later on.

Botilinum toxin injections better known as Botox, a leading brand from US company, Allergan.  There are other brands out there such as Dysport made by French manufacturers, Ipsen.

How does botox work?

Botilinum toxin is a purified protein that blocks the nerve signal from the brain to the nerve endings (that tell you to frown).  Injecting botox effectively paralyses the muscles, so wrinkles, lines and furrows don’t have the strength to smooth out.

What is botox good for?

Crow’s feet, forehead lines, lines around the mouth, upper lip lines, neck wrinkles, deep lines in the face and dimpling on chin.

What happens when the botox is injected?

Botox is injected into problematic areas using a very fine needle.  You may require several injections depending on the extent of the problem.

How does the botox treatment feel?

You will feel pinpricks but it isn’t really painful.

From ten to twenty minutes.

How long before it heals?  

Immediately.  You can go straight back to your life.  However, we do advise avoid facials and massages for 24 – 48 hours after the treatment which will move the product.

Over the last few years, we notice the demand for plastic surgery increasing for women spanning larger social and age groups; our non-invasive cosmetic surgery procedures such as botox and fillers has tripled; and in our LCAS London office the number of men coming in for surgery has also quadrupled.  In many of these social groups we come across, plastic surgery procedures can be as common as having your hair-dyed.

Are some plastic surgery procedures becoming more popular or are different types of women now requesting surgery?

We would say a little bit of both.  VASER liposuction and breast augmentation (breast enlargements) are extremely popular plastic surgeries for us here at LCAS Dubai, and the biggest group of women coming to see us are the ‘Mummy’ category.  Most of the Mums that we see may have lost their confidence after having children because their families have been their priority and often their own needs come last including their bodies.  Then one day, they decide that enough is enough and they are determined to get back into shape with a little help from us.

Any statistics you can give us regarding plastic surgery, with particular reference to Dubai/the Middle East?

It is hard to give exact statistics but the trends are definitely there.  Men doing plastic surgery are on the rise. In total with both men and women, LCAS Dubai sees 50% locals and 50% expat; and with such a large expat community there are nationalities that were previously shy to the idea of cosmetic or plastic surgery are coming forward more and more to get procedures done

We must all be careful when looking for a cosmetic or plastic surgeon in Dubai, Abu Dhabi, or anywhere in the world including London.  Here are some facts to be aware of when thinking about doing a cosmetic surgery procedure.

We believe there are botched jobs all over the world, and the UAE is no exception.  As cosmetic surgery has become more readily available to the masses, more and more people are tempted by the procedures.   However, with so much information and choice out there most people don’t know where to start – it is extremely confusing.

In some groups of women, we see that cosmetic surgery and plastic surgery is treated like a candy store, or as easy as buying a lipstick over the counter.  However, patients must remember that they are still dealing with their bodies, and homework should be done.  The worst thing to do is to seek surgery or treatments only by price.  Of course, price is a consideration but that should not be the sole deciding factor.

Often we try to educate women and advise them that a plastic surgeon is the same as an artist – each will work a ‘canvas’ a different way.  Botched jobs can be cause by surgeons who do not have an aesthetic eye, and problems can be caused by poorly trained or even untrained surgeons.

We have seen problems with liposuction where the surgeon operated too superficially to the skin that it created waves on the skin’s surface, or we have seen too much fat removed from one side of the body compared to the other.  We have seen poor breast augmentation work when implants are far too large for the  breast pocket.  We have seen bad face lift work which has left patients partially paralyzed.   One common thing we find in the UAE is the problem of skin discolouration due to poor laser work.

For the latter, it is not because of the lack of legislation as the UAE has rules and regulations especially Dubai Healthcare City which has even more stringent rules, but there are these ‘untrained professionals’ operating under the radar that is hard to control.  Often, nothing is heard about these in the media until something goes wrong.  For example, that unlicensed surgeon in Al Barsha was only caught when something went wrong.

However, there are always unethical plastic surgeons and cosmetic surgeons agreeing to perform unnecessary surgery on patients, and that can lead to problems too.  Sometimes doctors agree to perform surgical procedures which are not their strength.

How to look for a good plastic surgeon in Dubai?

In looking for a good plastic surgeon in the UAE, it is a small community, and most people know good surgeons and doctors.  The best way to find a good doctor is by referral.  Often the same recommendations come up, and then it is important to see 2 – 3 surgeons or doctors.  After seeing a few surgeons you will usually get a better understanding of the procedure that you are about to undertake.  Ensure that you are comfortable with your surgeon, and a good surgeon will always take the time to explain to you the benefits as well as the risks.  Every surgery carries risks, and it is important that the patients are aware as at the end of the day they should make a decision that is best for them.

1.       What is the most common surgery procedure you perform on men?

Definitely VASER liposuction.  That is our most popular surgery for men.  Very typically, when men age and they work longer hours whilst having busy family lives their bodies take a toll, and they are prone to putting on weight especially around the ‘man-boobs’, abdomen area and their flanks (lower back).  They end up with a common fat ‘tyre’ around their waist, and it has also been proven medically that this ‘tyre’ also represents what is happening internally.  Dr Mike Knapton of the British Heart Foundation said in a recent interview with the BBC ‘that the ‘harmless’ spare tyre around your waist is actually a major health hazard’.  We try to help men change their lifestyles completely from eating better and exercising more post-surgery.  Also, men want to get back their once youthful physiques so we can perform a high definition surgery that enhances the appearance of the six-pack.  With a little exercise, these muscles are even more apparent and impressive. Our second most popular operation is the penoplasty, where we take the fat from the liposuction and insert it in the area to increase the width – we have been performing this procedure for the past 20 years, and were the first to perform this surgery in Europe.

2.       Do you still get more women than men coming to you for help with plastic surgery?

In our LCAS Harley Street clinic, London, we have a ratio of 60%women to 40% men.  The number of men coming to see us in increasing exponentially.  In our clinic in LCAS Dubai based in Dubai Healthcare City, we see 70% women and 30% men.

3.       Which issue is a bigger concern amongst your plastic surgery male clients, is it age of obesity?

Both really.  We do have some men who look after themselves extremely well, and they add botox and fillers to their anti-ageing regime.  Women are looking better these days as they look after themselves, and similarly men don’t like to age either.  Then, there are other men who are very concerned with being very over weight as it affects their confidence, and sometimes it can lead to depression.  These patients agree to have their VASER liposuction with us, and it is amazing to see the change in them as their once youthful physiques are restored.  After VASER liposuction, our male patients are sometimes more determined than our female patients to change their lifestyles to maintain the results.
